The Jewish Association on Aging - a progressive non-profit health care organization located in the Homestead/Squirrel Hill area - has a casual Occupational Therapist position available.
JOB SUMMARY:
The occupational therapist is a licensed professional responsible for evaluating, planning, and implementing occupational therapy service to patients in a home health setting as prescribed by the physician. The therapist works as a member of an interdisciplinary team.
JOB DUTIES / RESPONSIBILITIES:
- Providing excellent customer service to the residents and to coworkers by demonstrating respect and concern.
- Performs comprehensive evaluation of patients by assessing level of function with activities of daily living.
- Develops plan of care cooperatively with the physician, patient and family with revisions made based on patient condition.
- Implements the therapeutic actions of the treatment plan of care as prescribed by the physician to restore function to patients with physical disabilities, to relieve pain, and to address associated psychological dysfunction in order to regain maximum independence in independent living/daily living skills.
- Instructs patients, families, and other health care team members in the care of patients regarding occupational therapy goals.
- Supervises assigned occupational therapy assistants in the treatment of patients.
- Plans with the patient, family, and other team members for the patient’s discharge from service.
- Completes all documentation on a timely basis which includes but is not limited to the initial evaluation forms, progress notes, plans of care, and discharge notes.
- Prepares patient’s record for recertification if necessary.
- Reports to the interdisciplinary team (IDT) a summary of the patient’s condition and any other pertinent information.
- Works in conjunction with the clinical supervisor to schedule all appointments as needed.
- Adheres to all policies set forth in the Occupational Therapy Practice Act and relevant professional practice standards to maintain safe clinical practice.
- Maintains a professional standard of conduct, projecting a positive image of the agency at all times.
- Complies with infection control and safety policies and procedures.
- Adheres to all policies and practices of the agency to maintain established Standard of Care.
- Performs additional duties as assigned.
REQUIRED EDUCATION, TRAINING and EXPERIENCE:
- Graduate of a state approved school of occupational therapy accredited by American Occupational Therapy Association and licensed as an occupational therapist in the Commonwealth of Pennsylvania.
