The Jewish Association on Aging is hiring a Hospice Aide!

The Hospice Aide provides services for patients with functional impairments who require assistance with their activities of daily living.

Job Summary:

· Provides direct personal care to patients as directed by the Registered Nurse

· May include: bathing, oral care, peri-care, shampooing hair, shaving, dressing, ROM, assisting with ambulation and getting patient in and out of bed.

· Performs simple household service, which facilitates self-care such as vacuuming, dusting, cleaning bathroom, and changing linens.

· Demonstrates skills necessary for family caregivers to meet patient personal hygiene, skin care and exercise needs.

· Teaches patient/family personal care, body mechanics, transfer techniques, safe environment, and infection control.

Job Duties and Responsibilities:

· Works in harmony with other members of the interdisciplinary team.

· Provides direct personal care to patients as directed by the Registered Nurse.

· Teaches patient/family personal care, body mechanics, transfer techniques, safe environment, and infection control.

· Understands the hospice & palliative care, mission, philosophy and services.

· Follows basic infection control policies, including: washing hands before and after caring for each patient, handles linens appropriately, and properly disposes of waste materials.

  • Maintains confidentiality of information relating to patients. Will discuss only those aspects necessary for care and treatment of the patients with those directly involved in the patient’s care per HIPPA regulations.

· Checks in and reports presence to various facility staff prior to starting patient care and notifies staff when leaving facility.

· Records visit and signs original signature on all progress notes.

· Attends Interdisciplinary Group (IDT) meetings as required by supervisor.

· Maintains current and accurate patient visit and mileage records.

· Compiles with the JAA dress code.

· Carries out other assignments as requested.

· Attends mandatory in-services and meets minimum requirements for yearly in-service hours to keep current on hospice & palliative care related issues and other professional topics.

· Exhibits active listening skills and practices not imposing personal beliefs on others.

· Approaches patients and families in a non-judgmental manner.

· Uses proper body mechanics and procedures for patient care minimize injury to patient and self.

· Uses appropriate personal protective equipment to accomplish all tasks and aspects of patient care.

· Maintains personal nurse care bag with all required items to provide immediate patient care and sees items are used are replaced by the start of the next day.

· Consistently follows “Call-on” and “Call-off” procedures to maintain awareness of daily schedule so that changes can be disseminated to avoid repetitive and/or uninterrupted patient service.

· Projects and exhibits professionalism, courtesy, and proper ethical conduct to the public, patients and other staff at all times to best represent the organization.

· Takes accurate vital signs and records as directed by the Registered Nurse.

Required Education, Training:

· High School Diploma/GED

· Certified as a Home Health Aide in the state of Pennsylvania and able to work in a hospice & palliative setting.

Required Skills and Abilities:

· Ability to be flexible and communicate effectively within a team and have concern for the provision of quality services.

· Ability to read and write English and to follow written schedules and instructions.

· Ability to work without direct supervision.

· Demonstrate empathy and maintain confidentiality of agency and patient information.

· Valid driver’s license, car insurance and car required
