Job Summary:
The Personal Care Division Resident Advocate provides support/coordination of services to residents and families to ensure their social, emotional, economic and spiritual needs are met by the community. This position is also responsible for maintaining regulatory compliance regarding required admission and annual paperwork as well as appropriate documentation.
Job Duties/Responsibilities:
- Complete required admission paperwork timely, in accordance with Department of Human Services Regulations and community policy
- Complete required annual paperwork in accordance with Department of Human Services regulations and community policy .
- Attend and coordinate / schedule Support Plan meetings in accordance with Department of Human Services regulations.
- Assist residents and their families through the admission/transition process by offering emotional support and guidance and coordination of services as needed.
- When appropriate, assist residents and families with the discharge process and transition to a higher level of care or to discharge home.
- Consult and coordinate with other members of the interdisciplinary team and outside agency community supports regarding the social, emotional, spiritual and economic needs of the residents.
- Act as an advocate for residents and their families.
- Provides/ coordinates with outside agency supports for short term crisis intervention for individuals or family counseling as needed.
- Identify the need for referrals to an outside agency and assist with those referrals.
- Participate in Resident and Family Council meetings as scheduled.
- Acts as a liaison between psychiatric physicians, nurses, residents, families and facility staff.
- Provide education and training to staff in the area of Resident Rights and related issues.
Required Education, Training and Experience:
1. BSW or related degree.
2. Minimum of 3 years experience working with the elderly population, facility
experience preferred.
3. Must meet Commonwealth of Pennsylvania requirements for job responsibilities.
Required Skills and Abilities:
1. Effective communicator with excellent written and verbal skills.
2. Effective organizational skills.
3. Able to monitor or oversee multiple projects.
4. Strong documentation/record keeping skills.
5. Awareness of DPW/DHS/PCH regulations.
