We currently have an exciting opportunity for a PRN/Casual Physical Therapist in our Home Health division.
Job Summary
The Physical Therapist is a licensed professional responsible for evaluating, planning and implementing physical therapy service to patients in an home health setting as prescribed by the physician. The therapist works as a member of an interdisciplinary rehabilitation team.
Job Duties/Responsibilities
- Performs comprehensive evaluation of patients by applying ROM, strength, coordination, balance and functional mobility tests.
- Develops the plan of care cooperatively with the physician, patient and family with revisions made based on patient condition.
- Provides therapeutic actions to relieve pain and/or develop or restore function to regain maximum independence.
- Instructs patients, families, and other health care team members in the physical therapy program, in the proper and safe use of prosthetic, orthotic and assistive devices, and safe transfer activities.
- Supervises assigned physical therapy assistants and aides in the treatment of patients.
- Plans with the patient, family, and other team members for the patient’s discharge from service.
- Collects OASIS data and completes all documentation in electronic medical record on a timely basis which includes but is not limited to the initial screening/evaluation forms, progress notes, care plans, and discharge notes.
- Prepares patient’s record for recertification if necessary.
- Reports to the case manager a summary of the patient’s condition and any other pertinent information.
- Works in conjunction with the intake coordinator to schedule home health physical therapy visits as indicated.
- Adheres to all policies set forth in the Physical Therapist Practice Act and to Physical Therapy Practice Standards to maintain safe clinical practice.
- Maintains a professional standard of conduct, projecting a positive image of agency at all times.
- Complies with infection control and safety policies and procedures.
- Adheres to all policies and practices of the agency to maintain established Standards of Care.
- Performs additional duties as assigned.
Required Education, Training, and Experience
Graduate of state approved school of physical therapy accredited by the American Physical Therapy Association and licensed as a physical therapist in the Commonwealth of Pennsylvania.
Required Skills and Abilities
- Ability to input billing information into computer
- Excellent interpersonal skills
- Knowledge of basic evaluation and treatment documentation skills.
