Principal Responsibilities:
Co-op Village NORC (Naturally Occurring Retirement Communities) is an innovative Older Adult Program which provides comprehensive and responsive social work and nursing services to assist older adults to age in place with dignity and independence through the facilitation of tailored supportive services. Through holistic wellness, cultural and social programs, NORC creates a community of engagement, connection and belonging for older adults.
We are seeking an experienced Senior Social Worker who will maintain a caseload of older adult clients, including assessment of client's core strengths, needs, and challenges, provision of case assistance, supportive counselling services, and group services within this context.
RESPONSIBILITIES
- Maintain a caseload of older adult clients, providing comprehensive assessment of needs, case management, supportive counselling and routine follow-up.
- Formulate ongoing care plans based on clients' challenges, needs, and strengths, and make referrals to external agencies and appropriate services. Connect client to applicable resources and intervention strategies. Monitor and evaluate effectiveness of intervention
- Work collaboratively with on-site nurses and interdisciplinary team to ensure consistency of service to clients
- Oversee Senior Companions Friendly Visiting Program
- Participate in biweekly case conference
- Attend supervisory, agency and community meetings as required
- Maintain up-to-date case records and program statistics in documentation database
- Facilitate and run a minimum of one activity, group or program per month
- Assist with organizing NORC and OAS events as needed.
- Provide program coverage for NORC activities and events as needed
- Work one Sunday per year for NORC Annual Funding-Raising Event
- Perform other related duties as assigned by Director and/or Senior director
Minimum Qualifications:
QUALIFICATIONS
- LMSW. Minimum of 1 year experience working with older adults and/or in social services is required.
- Knowledge of resources and entitlements, as well as knowledge of age-related cognitive and mental impairments required.
- Attention to detail, ability to work under pressure, and effectively coordinate a comprehensive array of services to address the multiple needs of the NORC population.
- Ability to take initiative and work independently.
- Prioritize tasks and cases, demonstrating astute judgement and time management skills.
- Utilize supervision effectively, maintaining a growth mindset by asking questions, collaborating with others, and insightfully reflecting on social work practices with older adult community members.
- Ability to maintain confidentiality of records and information.
- Must have strong verbal and written communication skills.
- Must work as collaboratively as part of the NORC and OAS team.
- Knowledge of MS Office (Outlook, Word, Excel) is required.
Salary:
$65,000 - $75,000
