**Families facing pediatric illness are navigating one of the most difficult chapters of their lives. The Case Manager is often the person who stands beside them through it all – helping them think clearly in moments of crisis, connecting them to resources they didn’t know existed, and ensuring they never feel alone in the process. This role is deeply relational and highly dynamic.

This is not a traditional desk job. One day might involve sitting with a parent who needs guidance navigating a new diagnosis; another might involve coordinating services, meeting with community partners, or helping a family access a critical resource. The Case Manager will serve as the primary support professional for approximately 50 families across the Washington, D.C. and Greater Washington region, ensuring each family receives thoughtful, consistent, and personalized support.

(3) Empowerment: You equip families with the tools, information, and confidence they need to navigate their journey with clarity and strength. Through thoughtful case management and steady advocacy, you help families understand their options, access the right resources, and make informed decisions that support their child and the well-being of the entire family.
(4) Community: You ensure that every family feels supported and seen throughout their experience, approaching each relationship with empathy, consistency, and care. You build meaningful, trusting connections with families through regular engagement and thoughtful follow-up, while also strengthening the broader network of support around them. By cultivating relationships with schools, community organizations, and healthcare providers, you help create pathways to resources and collaboration. Through these efforts, you play a key role in expanding imadi’s presence, partnerships, and impact throughout the D.C. and Greater Washington region.

REQUIRED EXPERIENCE

A master’s degree is preferred but not required, and we’re flexible on the discipline - we’re looking for someone who is compassionate, resourceful, hardworking, and highly proactive.  Ideal candidates come from backgrounds such as social work, human services, public health, healthcare navigation, or community-based support programs.
