WHAT YOU WILL DO:

The Lead Medical Coordinator is an exempt position responsible for overseeing and coordinating the provision of timely, appropriate health services for children in care. Additionally, the position is responsible for developing and maintaining policies and procedures for confidential health record management, triage of health concerns, medical emergency response, environmental health and safety, and post-release health care planning/care coordination.

Supervisor Responsibilities:

  • Oversee the Medical Coordinators
  • Provide staff training for medical, dental, and health topics relevant to pediatric, adolescent, and migrant health.

Duties and Responsibilities:

  • Ensure all UCs are seen by a medical provider by scheduling and confirming Initial Medical Exam (IME) appointments. This includes transporting children to their medical appointments, completing the IME form, and making copies of necessary clinical information from the client's chart.
  • Ensure accurate documentation of medication regimens for all children who need such services. Track medication logbooks to ensure information is completed accurately.
  • Attend meetings as assigned by supervisor, including staff meetings and compliance training programs.
  • Work on an as-needed basis during emergency conditions.
  • Provide emergency medical assistance care for clients as necessary.
  • Take inventory of over-the-counter medication, prescription medications, and personal protective equipment that are available and what is needed.
  • Assisting with initial medical screening of each child at intake by performing rapid COVID-19 tests on UCs upon admission and any other exam required, and documenting/reporting results in order to determine quarantine status.
  • Ensure concise and timely completion of all paperwork or documentation, including but not limited to administrative, clinical, or caseload requirements.
  • Accountable for the maintenance of portal entries for Treatment Authorization Request (TAR), medical complaints, and all medical information as necessary.
  • Review and maintain current medical information on all children, including taking vital signs as needed.
  • Act as liaison between physicians/nurses and children.
  • Perform the duties of the Medical Coordinator in case of absence.
  • Perform all other duties as assigned.

WHAT YOU WILL NEED:

  • Licensed Registered Nurse (RN) with a Bachelor’s degree;
    • Or Higher health-related qualifications (e.g., Physician Assistant, Nurse Practitioner)
  • Minimum of one (1) year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
  • One (1) year of supervisory experience is preferred
  • Proficient with the Microsoft Office Suite or similar software.
  • Strong oral and written communication skills.
  • Ability to form alliances with providers/organizations.
  • Ability to coordinate efforts, groups, company requirements, and activities to achieve program goals
  • Potential exposure to communicable diseases and blood-borne pathogens.
  • Be familiar with the location of the facility's fire alarms, extinguishers, exits, protective equipment, and first-aid kits. Be familiar with the facility's evacuation routes, exposure control, and disaster plans.
  • Endure short, intermittent, and/or prolonged periods of sitting and/or standing in performance of job duties.
  • Ability to work under stressful situations.
  • Proof of Immunization for: Varicella (2 doses), MMR (2 doses), Influenza (1 dose yearly), Tdap (1 dose).
  • Bilingual: English/Spanish, strongly preferred
  • May be required to use a personal vehicle to transport clients. Must be able to drive the company van as needed to transport clients.
  • Must have a valid driver’s license with auto insurance coverage.
