WHAT YOU WILL DO:
The AFP Case Worker is a non-exempt position responsible for working with clients with Serious and Persistent Mental Health (SPMI) and Caregivers who provide housing. The AFP Case Worker is accountable for assessing clients, facilitating placement with assigned Caregivers, and ongoing monitoring of that placement.
Must be able to travel and work in Alachua County and the surrounding areas.
- Complete a minimum of monthly face-to-face home visits in the community, which include assessing the safety and well-being of enrolled clients, the safety of the home, and compliance with program requirements.
- Maintain an active caseload/census and electronic health records for assigned clients.
- Evaluate referred clients for appropriateness of program and refer for services.
- Establish rapport with the client, caregiver, family/guardian, significant other, and all service providers involved with the client.
- Utilize de-escalation skills and training to ensure client safety, including developing service and safety plan(s).
- Complete agency and funder reports.
- Perform other duties as assigned.

WHAT YOU WILL NEED:
- Bachelor’s degree in human services or related field.
- Previous experience with Mental Health/ Substance Abuse.
- Computer experience, which includes Microsoft Office and Electronic Health Records.
- Must possess strong oral and written communication skills.
- Excellent attention to detail and organization skills.
- Must obtain/ maintain CPR and first-aid training within 90 days of employment.
- Ability to work under stressful conditions.?
- Ability to lift and/or carry supplies, files, etc., up to 25 lbs
- Must possess a valid driver’s license and current auto insurance coverage.
- Must use personal vehicle for community-based visits. Must be able to travel and work in Alachua County and the surrounding areas.
