WHAT YOU WILL DO:
The AFP Case Worker is a non-exempt position responsible for working with clients with Serious and Persistent Mental Health (SPMI) and Caregivers who provide housing. The AFP Case Worker is accountable for assessing clients, facilitating placement with assigned Caregivers, and monitoring placements on an ongoing basis. The case worker must travel to home visits in Flagler County and the surrounding areas.
- Complete monthly home visits in the community, which include, but are not limited to, assessing the safety and well-being of enrolled clients, the safety of the home, and compliance with program requirements.
- Maintain an active caseload/census and electronic health records for assigned clients
- Evaluate referred clients for appropriateness of program and refer for services
- Establish a rapport with the client, caregiver, family/guardian, significant other, and all service providers connected to the client
- Schedule and transport clients to community appointments
- Travel to home visits in Flagler County and surrounding areas
- Utilize de-escalation skills and training to ensure client safety, including the development of a safety plan
- Complete agency and funder reports
- Perform other duties as assigned

WHAT YOU WILL NEED:
- Bachelor's degree in the human service field with two (2) years' experience.
- Previous experience with SPMI and/or Co-occurring population preferred
- Must obtain and maintain CPR and First Aid training
- Must participate in all mandatory Department of Children and Families (DCF) and agency training each year.
- Competency in Microsoft Office Suites and Electronic Health Records platforms
- Must have strong oral and written communication skills
- Must use personal vehicle to travel to home visits in Flagler County and surrounding areas.
- Must have a valid driver's license with auto insurance coverage.
