WHAT YOU WILL DO:

The Clinical Support Coordinator is a non-exempt position responsible for liaising with behavioral health partners and providers, the funder utilization management team, and the criminal justice system for clients with a mental health diagnosis transitioning through community residential treatment.

  • Utilize the funder portal to review referrals, screen potential residential clients, and complete dispositions.
  • Participate in the admission intake process to identify the client’s needs.
  • Provide case management to assigned Adult Residential Treatment and Stabilization (ARTS) clients.
  • Maintain communication with the client’s internal/external treatment team, advocate for the client's needs, assist in linking with needed services/resources, and coordinate discharge transition.
  • Assist clients with resource and benefit applications, attend scheduled appointments, and follow up as required.
  • Liaise between the mental health system and legal system for ARTS clients with forensic involvement.
  • Maintain a calendar of all scheduled court hearings, competency restoration classes, probation appointments, and any additional court-related appointments.
  • Attend all scheduled hearings (in person and/or online), represent the ARTS Program, and provide testimony when required.
  • Complete status reports for court hearings using the agency's Electronic Health Record (EHR) and follow-up progress reports to the court/funder.
  • Participate in treatment team meetings to ensure the client is progressing towards the identified discharge plan.
  • Attend case staffing with the funder utilization management team when needed.
  • Will be required to transport ARTS clients in agency vehicles when needed.
  • Perform all other duties as assigned.

WHAT YOU WILL NEED:

  • Bachelor’s degree in Social Work, Mental Health Counseling, Psychology, or related human service field.
  • One (1) year of experience with Severe and Persistent Mental Illness (SPMI), co-occurring disorders, and or forensic adults strongly preferred.
  • Experience in mental health treatment, particularly related to justice-involved populations.
  • Working knowledge of community organizations and resources.
  • Excellent organizational skills and attention to detail.
  • Ability to maintain and develop relationships with community agencies and referral sources.
  • Excellent written and verbal communication skills.
  • Ability to conduct formal and informal meetings.
  • Proficiency with Electronic Health Record (EHR) Systems or the ability to quickly learn the EHR system to document client information in a clear and concise manner.
  • Ability to maintain confidentiality, prioritize work assignments, and work independently.
  • Proficient in Microsoft Office Suite and virtual meeting platforms.
  • Flexible work schedule in the office.
  • Ability to work overtime when needed.
  • Ability to work in the field in both indoor and outdoor environments.
  • Ability to stand, bend, lift, and move frequently around a large property for prolonged periods of time.
  • Must have a valid driver's license with current auto insurance coverage.
