WHAT YOU WILL DO:

The Hub Navigator is a non-exempt position responsible for providing assisted outreach services, in partnership with The Well for Life, to adults, children, and families. The Navigator may provide trauma-informed engagement, counseling, referral, and linkage to formal (community providers) and informal (family, friends, neighbors, school, etc.) resources to prevent ongoing client crises.

  • Provide an in-person response to referrals as required and within the periods designated by contractual requirements and/or program procedure
  • Engage with adults, children, and families, as well as their informal support networks (friends, relatives, neighbors, etc.)
  • Identify strengths, limitations, risk factors, and needs, using the required assessment tool to develop a Care Plan and coordinate services
  • Provide information, referral, and follow-up for appropriate ancillary community counseling, psychiatric services, financial, employment, parenting, housing, and other services as deemed appropriate to meet client needs
  • Ensure concise and timely completion of all documentation according to contract requirements, including entering all case activities into the AVATAR within two (2) business days
  • Perform all other duties as assigned.

WHAT YOU WILL NEED:

  • Bachelor’s Degree from an accredited college/university with two (2) years’ experience working with at-risk populations to include families experiencing substance abuse matters, domestic violence, mental health, and serious emotional disturbance.
  • Thorough knowledge of human development, counseling theories, and practices.
  • Knowledge of community organizations and resources.
  • Ability to perform individual, family, and group counseling sessions independently.
  • Ability to work under stressful situations.
  • Ability to work overtime when required.
  • Potential exposure to clients who may be verbally and/or physically aggressive.
  • May be required to use personal vehicle for client transportation.
  • Must have a valid driver's license with auto insurance coverage.
