WHAT YOU WILL DO:

The AFP Case Worker is a non-exempt position responsible for working with clients with Serious and Persistent Mental Health (SPMI) and Caregivers who provide housing. The AFP Case Worker is accountable for assessing clients, facilitating placement with assigned Caregivers and ongoing monitoring of that placement. Must travel to home visits in Flagler County and surrounding areas.

  • Complete monthly home visits in the community which includes but not limited to assessing the safety and well-being of enrolled clients, safety of the home and compliance with program requirements.
  • Maintain an active caseload/census and electronic health records for assigned clients
  • Evaluate referred clients for appropriateness of program and refer for services
  • Establish a rapport with the client, Caregiver, family/guardian, significant other, and all service providers connected to the client
  • Schedule and transport clients to community appointments
  • Must travel to home visits in Flagler County and surrounding areas.
  • Utilize de-escalation skills and training to ensure client safety including the development of a safety plan
  • Complete agency and funder reports
  • Perform other duties as assigned

WHAT YOU WILL NEED:

  • Bachelor's degree in the human service field with two (2) years' experience.
  • Must have a current certification in CPR and First Aid.
  • Must participate in all mandatory Department Children and Families (DCF), and the agency training each year.
  • Must travel to home visits in Flagler County and surrounding areas.
  • Must have a valid drivers' license and proof auto insurance coverage.
