GENERAL JOB DESCRIPTION

The Case Manager plays a central role in helping individuals with disabilities develop, implement, and sustain employment and skill building plans that reflect their interests, strengths, and goals. Case Managers also train the Supervisors and direct care staff on participant plans and develop training strategies to assist participants in reaching their goals. This role requires problem solving while maintaining case management compliance, quality, and continuity of services.

QUALIFICATIONS

Education:

Bachelor's degree in psychology, social work, special education or related field required.

Other:

  • 21+ years of age.
  • Criminal background must meet funding source requirements.
  • Reliable transportation and valid Michigan driver’s license.
  • Driving record must be approved by Agency insurance carrier.
  • Complete all assigned trainings including but not limited to CPR, First Aid, and Recipient Rights.
  • Two years related work experience serving individuals with disabilities required.
  • Strong interpersonal skills to communicate clearly and professionally with participants and staff.
  • Composition skills sufficient to prepare required documentation.
  • Fluency in American Sign Language skills is a plus.
  • Basic computer skills.

DUTIES AND RESPONSIBILITIES

  1. In conjunction with the participants and staff, develop and implement specific programs to assist participants with learning and maintaining skills that maximize and promote a healthy, active and socially integrated lifestyle.
  2. Using a person-centered approach, provide case coordination, follow-along supports, linkages and advocacy services to participants
  3. Manage day-to-day operations of assigned employment, volunteer/and or recreations locations, including developing, implementing, and maintaining quality procedures so that service and safety requirements are met.
  4. Develop, monitor, and periodically review support plans for designated participants.
  5. Secure and maintain required authorizations for service.
  6. Prepare and submit necessary documentation in computerized databases for funding source in a timely manner.
  7. Generate reports and other documents to ensure that the case record is in accordance with all applicable regulations for governing accreditation and funding bodies.
  8. Carry cell phone and be available to provide coverage to ensure proper staffing at sites, as well as to provide direction to staff.
  9. Establish and maintain positive and outcome-oriented relationships with participants and their involved others, community partners, other team members, and funding sources.
  10. Perform job-coaching, and/or Lead Job Coach job duties, as needed.
  11. Transport participants using Agency vehicle or own vehicle, as needed.
  12. Perform other duties as assigned.
  13. Serve on Agency committee(s) as appointed.

WORKING CONDITIONS

Environmental conditions:

  • Moderate noise (i.e., business office with computers, phone, and printers, light traffic).
  • Ability to work in a confined area.
  • Ability to sit at a computer terminal for an extended period.

Physical requirements:

  • While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard, reach, stoop, kneel to install computer equipment.
  • Specific vision abilities required by this job include close vision requirements due to computer work.
  • Light to moderate lifting in required.
