Community Health Worker (CHW) – 3 Open Positions

Salary: $50,000–$55,000 annually

Schedule: Flexible full-time schedule; may include Monday–Friday or Tuesday–Saturday, with occasional evenings as needed

The Barry & Florence Friedberg JCC is seeking three compassionate, motivated, and community-minded Community Health Workers (CHWs) to join our growing team. This is an exciting opportunity for individuals who are passionate about improving health equity and helping individuals and families access critical community resources and support services. Recent graduates are encouraged to apply.

As part of the Health Empowerment Alliance of Long Island (HEALI), Long Island’s Social Care Network, Community Health Workers help identify health-related social needs, connect community members to services, and support them in navigating complex systems of care.

This is a non-clinical, community-based role focused on outreach, advocacy, care coordination, and resource navigation.

Key Responsibilities

• Conduct screenings and interviews to identify health-related social needs
• Connect individuals and families to community resources and supportive services
• Assist clients with referrals related to food, housing, transportation, healthcare, and other social supports
• Follow up with clients to help ensure needs are addressed and barriers are reduced
• Build trusting relationships with clients from diverse cultural and socioeconomic backgrounds
• Document screenings, referrals, and case notes in required systems, including Unite Us
• Collaborate with healthcare providers, social workers, and community partners
• Provide education and guidance to help clients access benefits and services
• Maintain accurate and timely documentation
• Participate in team meetings, trainings, and professional development opportunities

Qualifications

• Bachelor’s degree required in social work, public health, psychology, human services, community health, or a related field
• Relevant internship, volunteer, or work experience in community health, social services, or human services preferred
• Recent graduates are encouraged to apply
• Excellent communication and interpersonal skills
• Strong organizational skills and attention to detail
• Ability to manage multiple tasks in a fast-paced environment
• Comfortable using technology, including Microsoft Office and web-based platforms
• Ability to work independently and collaboratively

• Valid driver’s license and reliable transportation required for local travel

Location Requirement

This position is primarily based in Southwest Nassau County and involves regular in-person community-based work. Minimal local travel may be required for outreach, meetings, and partner visits.

This is a grant-funded position currently scheduled through March 2027, with potential for extension based on funding.
