We are looking for an Admission/Discharge Coordinator to join our team here at Elan Skilled Nursing and Rehab!
This is a full-time, hourly position.

SUMMARY: The primary purpose of this position is to collaborate with the resident, family, and health care team to facilitate the process of transitioning the resident to their next setting. The position is assigned primarily to the short stay unit and assists social services team/business office.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Works in conjunction with the Director of Human Services in the admissions process of residents in the facility to ensure that Elan Skilled Nursing and Rehab is prepared to meet their needs. Share responsibilities of the administrative/clinical admission packet completion.
- Ensure room readiness for admissions: function of tv, DME, cleanliness of room.
- Develop relationships with residents and their families.
- Documents in PCC following up on home health referrals/DME start of care medical appointments and documents in PCC.
- Participates with dental services/transport and nursing as assigned.
- Maintains compliance with regulatory agencies in documenting assessments and progress.
- Maintains adherence to all Elan Skilled Nursing and Rehab’s policies and procedures and standards of governing bodies.
- Refers residents and family members to community resources.
- Participate in MA Application process on Mondays 1pm-4:30pm (other times as assigned by DHS)
- Uploads insurance cards/documents as directed by DHS
- Tracking of dental/eye consents/follow-up
- Emailing/communication board, care plan invitations and uploading to PCC-uploading all confirmations of HH
- Other duties as assigned

EDUCATION/LICENSURE: Must be able to utilize computer program of Microsoft Word and Excel

LANGUAGE SKILLS:
Ability to read, analyze, and interpret, professional journals, governmental regulations and procedure manuals. Ability to write reports such as documentation in medical records, business correspondence. Ability to effectively present information and respond to questions from groups of residents, family members, other staff members, and the general public.
REASONING ABILITY:
Ability to define problems collect data, establish facts, and draw valid conclusions. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
OTHER SKILLS and ABILITIES:
Strong interpersonal skills.
Conforms to established work hours and maintains an acceptable work attendance record.
