Description
SUMMARY: A Clinical Director is a Registered Nurse with Administrative and Clinical Leadership responsibility for an assigned neighborhood. The Clinical Director functions at a professional nursing level using both administrative leadership and clinical expertise in order to assure that quality, individualized, comprehensive care is rendered to the resident population, through collaboration with the interdisciplinary team. The Clinical Director has responsibility for continuity of care on the assigned neighborhood 24 hours per day.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Utilizes expertise in a leadership role as a Clinical Expert of an assigned neighborhood.
- Supervises and coordinates nursing services on assigned neighborhood.
- Coordinates staff scheduling to ensure resident care needs are met.
- Makes rounds on a regular basis to observe and monitor care being rendered.
- Assists with physician rounds as needed.
- Ensures nursing care is carried out as directed by practitioner orders and organizational policies and procedures.
- Evaluates the work performance of nursing personnel on all shifts as it relates to their job description and the standards and goals of the facility.
- Obtains report and communicates follow-up needs to nursing staff and supervisors.
- Demonstrates, teaches, and evaluates nursing skills utilized in the direct care of the neighborhood’s residents.
- Conducts regularly scheduled meetings with staff to establish neighborhood goals and protocols and to initiate, implement, and evaluate the effectiveness of these processes.
- Meets with evening and night supervisor when necessary to discuss concerns relating to their shift.
- Flexes schedule to directly observe and manage staff on all shifts and meet neighborhood goals and objectives.
- Identifies educational needs of the staff and plans programs to meet those needs.
- Participates in facility provided educational programs.
- Assures staff attend & receive training on resident care topics as required by regulations and resident care needs.
- Functions as Registered Nurse in Charge as assigned.
- Supervises, coordinates, monitors, and assists in orientation of new staff.
- Attends meetings as assigned.
- Participates in Quality Assurance/Performance Improvement activities as assigned.
- Adheres to facility policies & procedures for nursing practice.
- Understands and adheres to 49 Pa. Code § 21.18 Standards of nursing conduct.
- Understands and complies with 49 Pa. Code § 21.11 - §21.16 Responsibilities of the Registered Nurse.
- Understands and complies with State and Federal regulations for care of residents in nursing facilities.
- Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual needs and rights.
- Collaborates with Residents, Resident Representatives, Physicians and Physician extenders, and the Interdisciplinary team to coordinate individualized care based on Resident needs and goals.
- Reviews orders, consultant recommendations, and care plan changes and ensures care is appropriate and documentation is complete.
- Collaborates with RNAC in completion of MDS Assessments.
- Communicates with Residents and families to discuss Resident care needs and progress made towards goals; and provide encouragement and support to Residents and their families.
- III. Coordinates and communicates neighborhood activities both within the nursing department and the other departments in the facility.
- Meets daily with DON to discuss resident conditions.
- Acts as liaison between neighborhood and contract therapists to provide effective communication of needs and progress for residents receiving therapy services by:
- Coordinating requests for screenings, evaluations, and therapy orders
- Ensuring therapy schedules are met
- Monitoring resident progress and ensuring documentation is accurate and complete.
- Supervises Restorative nurse and ensures Restorative program documentation is complete and accurate.
- Coordinates and documents Restraint Reduction program for assigned neighborhood.
- Coordinates and documents Infection Control, Resident TB Surveillance, Influenza Campaign, and Antibiotic Stewardship program for assigned neighborhood.
- Coordinates and documents Incontinence program for assigned neighborhood.
- Coordinates and documents Falls & Incident/Accident program for assigned neighborhood.
- Coordinates and documents Wound program for assigned unit.
- Coordinates outside appointments for residents.
- Attends Quality of Life Rounds, Care Plan Meetings, and other meetings as assigned.
- Maintains and updates reports as assigned.
- Ensures that all required neighborhood records are current and complete.
- Participates in Quality Assurance/Performance Improvement activities as assigned.
- Delivers care to residents:
- Collects complete and ongoing data to determine nursing care needs.
- Analyzes health status of individuals and families and compares the data with norms when possible in determining nursing care needs.
- Identifies goals and plans for nursing care.
- Carries out nursing care actions which promote, maintain, and restore the well-being of individuals.
- Involves individuals and families in their health promotion, maintenance, and restoration.
- Evaluates the effectiveness of the quality of nursing care provided.
- Administers and evaluating the effect of medications and treatments.
- Delivers direct resident care following the Resident’s Individualized Care Plan. This includes, but is not limited to: assisting with meals, bathing, personal hygiene, oral hygiene, incontinence care, toileting, positioning, transferring, splint application, etc.
- Assessing residents as indicated by their status in accordance with facility procedures and standards of nursing practice.
- Documenting care delivered and assessments according to facility procedures.
- Ordering supplies, medications, and equipment as per facility policies and procedures.
- Accepts written and verbal orders for medications and therapeutic treatment from an authorized practitioner in accordance with facility policy.
- Questions any order that is perceived as unsafe or contraindicated for the resident or which is not clear and shall raise the issue with the ordering practitioner or the professional nurse in charge if the ordering practitioner is not available.
- Transcribes orders to the resident’s medical record per facility policy.
- Provides venipuncture: intravenous fluids when:
- The procedure has been ordered in writing for the resident by an authorized licensed practitioner.
- The Registered Nurse has had instruction and supervised practiced in administering parenteral fluids or medications into the vein.
- The intravenous fluid or medication to be administered is the fluid or medication specified in the written order.
- An accurate record is made concerning the following:
- The time of the injection
- The medication or fluid injected
- The amount of medication or fluid injected
- Reactions to the fluid
- Supervises Licensed Practical Nurses in the provision of venipuncture/intravenous fluid administration.
Serve as a clinical nursing consultant for Elan Gardens Personal Care Home.

SUPERVISORY RESPONSIBILITIES:
Manages and supervises employees in the assigned neighborhood. Responsible for the overall direction, coordination, and evaluation of assigned neighborhood. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE:
Registered Nurse licensed in the Commonwealth of Pennsylvania, with three to five years experience as a licensed nurse, preferably in long term care. Previous supervisory experience preferred.
Must maintain current Basic Cardiopulmonary Life Support (BCLS)
LANGUAGE SKILLS:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, residents, staff, family members and the general public.
REASONING ABILITY:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
CERTIFICATES, LICENSES, REGISTRATIONS:
Must posses a valid Pennsylvania nursing license.
Must maintain continuing education credits per licensure renewal requirements and resident care needs.
OTHER SKILLS and ABILITIES:
Strong organizational and interpersonal skills.
Conforms to established work hours and maintains an acceptable work attendance record.
Must be able to function independently.
