We are looking for a PT RN Admission Nurse for 2nd shift to join out dedicated team at Elan Skilled Nursing and Rehabilitation!

Pay Rate: $41-$46 per hour

Reports To: Director of Nursing

Job Summary

The skilled nursing center utilizes a registered nurse to be the first clinical point of contact for residents as they are admitted into our facility. Primary responsibilities include administrative tasks, such as collecting medical information, as well as providing resident care, as needed. The Admission Nurse may administer medicine, take vitals, educate and examine residents. The Admissions Nurse is responsible to report to the clinical team after gathering all pertinent information from interviews and assessments. All of this is to assure the highest level of quality care during the admission process and collection of all data that is required to develop the safest, complete plan of care for our residents.

Duties

  1. Coordinates the services of the Admissions Department with physicians for the most comprehensive resident care. Provides nursing care as may be required during the admission process.
  2. Establishes and maintains communication with all residents and family / significant others; and for smooth operations of admission. Gathers data relevant to the resident’s individual needs.
  3. Completes all job duties identified on Admission Checklist, while organizing the admission process for clinical staff. Completes medication reconciliation process with admitting physician, and receives physician orders to develop resident plan of care.
  4. Obtains a nursing history. Takes appropriate action when needed
  5. Discusses information regarding patient with physician as indicated. All interdepartmental actions initiated before the end of the admitting process (ie-Lab and or X-Ray sheets, Therapy communications, follow-up appointment scheduling, etc.)
  6. Reviews any abnormal diagnostic results from hospital procedures with admitting physician to better develop resident plan of care.
  7. Demonstrates a time conscious awareness and ability to prioritize. Distinguishes between low and high priority activities. Identifies and eliminates delays.
  8. Consistently is on time and ready for work at start of shift. Consistently completes all work within shift time frame. Does not allow outside commitments of personal issues to interfere with the work schedule
  9. Demonstrates a high level of mental and emotional tolerance sensitivity, diplomacy, sound judgment, and a professional attitude when relating with patients, families and co-workers
  10. Performs duties with frequent interruptions, in close quarters and under time constraints in a professional manner.

  1. Has knowledge of medical and technical terms used within the hospital. Maintains a current knowledge of the legal aspects of nursing and of the policies, procedures and regulations of the hospital. Clearly understands purposes, objectives, practices and procedures of department.
  2. Performs resident teaching presentations. Provides instructions and teaching to residents, as needed.
  3. Handles all situations with poise, understanding and tact. Promotes and maintains positive

relationships with staff, residents, families and general public. Is cooperative and open minded in working with others.

  1. Writes and maintains legibly accurate and current records, giving meticulous attention to detail. Creates appropriate notes in PCC EHR.
  2. Takes pride in personal appearance, displays the type of grooming which is neat, attractive and appropriate and projects a positive image.
  3. Demonstrates proper telephone techniques and etiquette. Uses proper oral and written language
  4. Seeks creative alternatives and innovative possibilities in problem solving. Demonstrates a strong ability to identify, analyze and solve problems and can be a relied upon to make sound decisions. Understands and considers the ramifications decisions may have on other departments.
  5. Is consistent, dependable and can be relied upon to meet schedules and deadlines.
  6. Effectively develops individual, departmental and organizational goals to obtain objectives.
  7. Excels in self-directing and self-pacing. Constantly seeks new and better ways of performing job.

Qualifications

Qualifications

  1. Must meet requirements of Registered Nurse job description
  2. Must have active CPR from American Heart Association
  3. Must demonstrate proficiency in medical terminology, abbreviations and spelling
  4. Ability to demonstrate use of electronic health record, photocopy machine, and other equipment post competency completion.
  5. Ability to use adult learning principles when teaching residents and families.
  6. Ability to prioritize and problem solve.
  7. Ability to relate well with physicians.

Work Schedule and Travel Requirements

This is a full-time position based at a designated location. Work hours are subject to variation in accordance with the operational requirements of the facility. Travel may be required solely at the discretion of the organization and only when deemed necessary.

Employees may also be subject to emotionally distraught residents, family members, visitors, and personnel. The noise level in the work environment is typically minimal to moderate.
