Social Worker

Elan Skilled Nursing and Rehab is looking for a Social Worker to join our dedicated team!

License not required

LTC experience required

Reports directly to Social Service Director

$50,000 - $55,000 per year. This is an hourly position.

Qualifications

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  1. Develops an on-going relationship with residents and their families.
  2. Interviews and assists residents and family members with problems such as personal and family adjustments, finances, physical and mental impairments.
  3. Secures information such as medical, psychological, and social factors contributing to resident’s situation, and evaluates these and resident’s capacities.
  4. Counsels residents individually, with family, or in other small groups regarding plans for meeting needs, and aids residents to mobilize inner capacities and environmental resources to improve social functioning.
  5. Participates in the Interdisciplinary Care Plan process on a regular basis and assures that all psychosocial problems, goals, and methods are identified and incorporated into the Interdisciplinary Care Plan. Reviews the Interdisciplinary Care Plan with residents and or family members as required.
  6. Maintains compliance with regulatory agencies in documenting assessments and progress notes along with entries of other relevant social service involvement in the residents care.
  7. Maintains, interprets, conveys and assures adherence to all Jewish Home policies and procedures and standards of governing bodies.
  8. Provides information, tours of the facility, and assistance to prospective residents and family members
  9. Establishes contact with local hospitals when a resident is temporarily transferred, and visits hospitalized residents as appropriate.
  10. Works closely with all staff members of the facility to identify those residents and family members in need of social service intervention.
  11. Refers residents and family members to community resources and other organizations.
  12. Assists residents at meal time as assigned.
  13. Assists in providing sensory stimulation to assigned residents as appropriate.
  14. Assist with and coordinate admissions as assigned by Director of Human Services including updating waiting list and medical abstracts.

EDUCATION/LICENSURE:

Hold a current license under [the Social Workers, Marriage and Family Therapists and Professional Counselors Act]; or hold a Bachelor's degree (B. A.) or a Master’s degree (M.S.W.) from four-year college or accredited school university in Social Work or social welfare.

LANGUAGE SKILLS:

Ability to read, analyze, and interpret, professional journals, governmental regulations and procedure manuals. Ability to write reports such as documentation in medical records, business correspondence. Ability to effectively present information and respond to questions from groups of residents, family members, other staff members, and the general public.

REASONING ABILITY:

Ability to define problems collect data, establish facts, and draw valid conclusions. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

OTHER SKILLS and ABILITIES:

Strong interpersonal skills.

Conforms with established work hours and maintains an acceptable work attendance record.
