Manny Cantor Center at Educational Alliance brings together a wide range of high quality, multi-generational programs and services to serve the Lower East Side community as well as other parts of NYC. As we have for 130 years, MCC offers best-in-class programming focusing on education, health & wellness, arts & culture, and civic engagement.
We are seeking a part-time Case Manager to work with our Community Programs and Social Services team. The Case Manager will provide case assessment, referral services and case management to a diverse community population with the aim of enhancing their well-being and socioeconomic mobility.
RESPONSIBILITIES
• Maintain a caseload of 10-15 clients, conducting intake assessments for public benefits and socioeconomic needs. Provide support by assisting with benefits applications, making referrals, and connecting clients to appropriate resources.
• Participate in regularly scheduled case conferences, staff meetings, and training sessions, while also providing assistance for cross-departmental events.
• Participate in community outreach events to promote the services offered by Community Programs and Social Services and establish networking connections.
• Maintain up-to-date case records and program statistics in Trax Solution database.
• Accompany clients to appointments for benefits and entitlement meetings when deemed appropriate.
• Other duties as assigned
REQUIREMENTS
- At least one year of experience working within a community-based organization
- Must be available to work Saturday shifts
- High School Diploma or equivalency
- Knowledge of available community resources, such as public benefits, housing assistance, healthcare services, employment support, and educational programs, and the ability to connect clients with appropriate resources.
- Excellent communication skills, both written and verbal.
- Proficiency in using MS Office applications such as Word, Excel, and PowerPoint.
