Organization Overview
The Commonpoint Workforce Services Department at “The Hub” includes programs that serve newly arrived migrants and all New Yorkers across the city. These programs focus on social integration, employment services, and community support, offering resources such as occupational training, education, and wraparound support for job-seeking adults and families motivated to enter the workforce.
The Connect2Care (C2C) Program, part of the Preventative Care Access Program led by the United Way of New York City (UWNYC), focuses on connecting at-risk, low-income New Yorkers to health and social services. The program seeks to improve access to medical homes and preventive care for chronic illnesses such as diabetes, hypertension, and obesity.
Key Responsibilities

Outreach & Engagement
Educate and build relationships with community members to promote participation in C2C activities.
Empower uninsured or underinsured individuals vulnerable to chronic conditions to engage in health and wellness activities.
Plan and host recurring health and wellness events, including lunch & learns health screenings, and outreach activities.

Care Coordination
Screen individuals for health and social determinants of health (SDOH) needs.
Refer individuals to appropriate health and social services and monitor referral outcomes.
Utilize the Connect UWNYC platform to track and manage referrals.

Communications & Network-Building
Collaborate with healthcare stakeholders to enhance visibility and promote health-centered community initiatives.
Partner with local organizations, including NYCHA developments, to co-host events and expand outreach.
Qualifications
Bachelor's degree or equivalent experience in community health, case management, health coaching, or care coordination.
Familiarity with the community being served and its services, with strong system navigation skills.
Reflective of the community demographics and/or actively engaged in the community.
Must be bilingual in Spanish and English
Proficiency in computer use is essential. Candidates must be tech-savvy and familiar with Google Workspace (Gmail, Google Docs, Google Sheets, Google Drive, etc.), as well as comfortable learning and using new technology platforms.
Training & Supervision
Participate in the C2C Health Advocacy Training Program alongside the Project Lead.
Submit timesheets and invoices to UWNYC monthly.
Support the Project Lead in program administration and implementation.
Location and Hours: Work will primarily be conducted in the community served by the program. A flexible schedule is required, with availability on weekdays and weekends.
Contract Duration: Until June 30, 2026
