Case Manager - Temporary

Chai Lifeline is seeking a compassionate and organized Temporary Case Manager to support the West Coast Division during a temporary staffing need. The Temporary Case Manager will coordinate, implement, and oversee case management services for families navigating chronic or life-threatening pediatric illness.
This role requires strong communication skills, cultural sensitivity, professionalism, and empathy. The Temporary Case Manager will help ensure families receive individualized support and access to Chai Lifeline's programs, resources, and services while maintaining the organization's commitment to integrity, compassion, collaboration, and respect.
RESPONSIBILITIES INCLUDE:
- Develop a thorough understanding of Chai Lifeline case management protocols as applied to the West Coast Division.
- Assess client needs and create individualized care plans tailored to each family.
- Coordinate and manage all aspects of assigned client care, ensuring timely follow-up and high-quality support.
- Build and maintain strong, trusting relationships with families in the West Coast community.
- Communicate regularly with parents, children, volunteers, schools, synagogues, hospitals, and community partners.
- Oversee logistical components of special gifts, services, and support initiatives for families.
- Participate in regional programs, home visits, events, and hospital visits to deepen relationships with families.
- Maintain accurate and organized documentation, case notes, and records.
- Bring creative ideas, resources, and program concepts to strengthen West Coast Division initiatives.
- Collaborate closely with the Director of the West Coast Division and the broader Chai Lifeline team to ensure seamless service delivery.
- Assist in ad hoc assignments

EXPERIENCE:
- 2+ years of relevant work or volunteer experience in case management, social services, youth or community support, mental health settings, or related fields

Position Details
This is a temporary, full-time position beginning in July 2026 and continuing through November 2026. Employment is expected to conclude at the end of the assignment period unless extended based on organizational needs.
